65 yrs old male, 5.5 months old operated case, Stiff joint ROM 0 -30 degree, co Pain on wt bearing, immediate post op n recent xray attached. Plan of treatment now?
Kunal BansalKindly revise the patient with longer and properly placed lcp with proper freshening and bone grafting with arthrolysis or pie crusting as required followed by Rom exercises immediately post operatively
Tribhuwan Narayan Singh GaurPie crusting percutaneous release of adhesion with wide bore needle, ln my view weight for union for 2-3 month, bone grafting can be done with manipulation but no need for revision
Ashwini SoodI think the problem is mainly due to 1 wrong position of a good implant.Plate seems to be placed anterolaterally which is preventing patella ROM. Solution :- Remove this plate .Can put a ordinary 8 xhole broad DCP as distal fragment can take atleast 3 screws + BG
Gopal GoelAssessment 1, plate is placed anterolaterally 2, plate is short . treatment — Remove the plate and either redo it properly or use supracondylar nail. you will also need adhesiolysis in suprapatellar area
Sadique Ahmed Khan1.nailing culd have been done.2. Plate put wrong way I.e. anterolateral n high up. Solutions. . 1.remove plate 2.free adhesions 3. For faster n better rehab do nailing n bg. Note. Plating can b done but will prefer nail for better rehab.
